The Licensed Trade Support&Care is founded on a long and successful history of care and offers a wide range of assistance to its beneficiaries. The charity will assist anyone who is, or has worked in the licensed drinks trade. Should you wish to apply for help please use one of the forms within this section.
Before completing the application forms, please read the following notes carefully.

Supporting Documentation

We will require proof of household income and savings with all applications.
To confirm your income we will accept photocopies of any of the following:

• A letter from the DWP/Pension Service giving a breakdown of your income
• 3 of your most recent payslips
• Most recent bank statement showing benefits/wages/pensions
• Most recent quarterly statement –for Post Office Direct Payment accounts
If you have any savings, please also include a photocopy of the most recent statement or latest page of the book if it is a book account.

Completing the form
Please complete all the sections of the form that apply to you.
You must provide your, and your spouse/partner's (if applicable) National Insurance Number. You will find your number on:
• State Benefit letters
• Wages slips
• Bank statements if you have benefits paid into your account

Work history
We will need proof of your work history within the licensed trade industry. Please send any documents that you have. This could include:
• Old payslips
• References
• Letters from previous employers
• Photographs of you at work
